Our current Minnesota Chapter 245D service lines are Community Residential Services and Integrated Community Supports. Supports within each program are individualized rather than offered as separate, stand-alone service lines.
Medicare-Certified Home Health
Our Home Health services include Skilled Nursing and Home Health Aide Services delivered in the patient’s home according to clinical need, applicable orders, eligibility, and an individualized plan of care.
