CRS and ICS inquiries may be initiated by case managers, care coordinators, social workers, discharge planners, individuals or legal representatives, counties, and other appropriate sources. Eligibility and service authorization must be established through the applicable waiver or lead-agency process.
Home Health inquiries may come from authorized practitioners, hospitals, rehabilitation facilities, clinics, care coordinators, discharge planners, patients or families, and other appropriate sources. Medicare-covered care remains subject to eligibility, required orders or certification, and the plan of care.
