How to initiate a grievance
A person who believes ElizJoseph Healthcare LLC subjected that person or another person to prohibited discrimination may initiate a grievance by calling 612-886-9404 or by mailing or delivering written correspondence to the Coordinator at the address above. A grievance will not be rejected merely because a person cannot submit it in writing or cannot provide every requested detail.
ElizJoseph Healthcare LLC will document and address grievances through a prompt and equitable process. Retaliation for filing a grievance, participating in a review, requesting assistance, or exercising protected rights is prohibited.
Conflict-of-interest route
If your grievance concerns the Coordinator or presents another conflict of interest, state that you need the Alternate Civil Rights Reviewer when calling, or mark written correspondence “Confidential—Alternate Civil Rights Reviewer.” The alternate reviewer’s personal contact information is not published.
