Skilled Nursing
When ordered, clinically appropriate, and included in the patient’s individualized plan of care, Skilled Nursing may include:
- Skilled assessment and monitoring
- Medication management, education, and monitoring
- Wound assessment and wound care
- Disease-process education and management
- Monitoring changes in condition and response to treatment
- Patient and caregiver education
- Care-plan implementation and coordination
- Communication with authorized practitioners and the care team
Home Health Aide Services
Home Health Aide Services may include personal-care assistance and other care performed according to the patient’s individualized plan of care and applicable supervision requirements.
Eligibility and availability
Home health services are based on clinical need, eligibility, payer requirements, applicable orders or referrals, and an individualized plan of care. Medicare-covered services require the applicable Medicare eligibility requirements to be met.
Services are provided in the patient’s home subject to service-area coverage, staffing, and current agency capacity. Skilled nursing visits are scheduled according to clinical need and orders; our on-call support is not continuous bedside nursing.
