A person’s own living unit

ICS is provided where the person has their own living unit. Housing and the HCBS service are distinct: support is delivered according to the individual’s assessed needs and authorized service plan.

Individualized supports may include

  • Community participation and social engagement
  • Health, safety, and wellness
  • Household management and adaptive skills
  • Activities of daily living when authorized
  • Medication assistance or support when authorized
  • Developing and maintaining independence
  • Coordination with the individual’s support team

Eligibility, scheduling, and capacity

ICS is for adults age 18 and older who are eligible and authorized through an applicable Minnesota HCBS waiver. Support is scheduled according to assessed needs and the authorized plan, with staff and on-call support available according to program requirements.

Admission is subject to assessment, authorization, setting compatibility, current capacity, and our ability to safely meet the person’s needs.
