Who may refer
CRS and ICS inquiries may be initiated by case managers, care coordinators, social workers, discharge planners, individuals or legal representatives, counties, and other appropriate sources. Eligibility and service authorization must be established through the applicable waiver or lead-agency process.
Home Health inquiries may come from authorized practitioners, hospitals, rehabilitation facilities, clinics, care coordinators, discharge planners, patients or families, and other appropriate sources. Medicare-covered care remains subject to eligibility, required orders or certification, and the plan of care.

The following information applies to our Medicare-certified home-health services. Coverage for CRS and ICS follows their separate eligibility and authorization processes.

ElizJoseph Healthcare LLC is a CHAP-accredited, Medicare-certified home health agency committed to nationally recognized standards for quality, safety, and patient-centered care.
CHAP accreditation became effective August 21, 2024 and expires August 21, 2027. Medicare certification became effective August 21, 2024.
