Support shaped around the person
Supports are provided according to each resident’s assessed needs and service plan. They may include:
- Activities of daily living and personal-care support
- Medication assistance or support when authorized
- Health, safety, wellness, household, and daily-living support
- Community participation and integration
- Behavioral and positive-support strategies when applicable
- Meal planning and preparation
- Transportation and community-access assistance when authorized
- Coordination with the person’s care team
- Support with maintaining or increasing independence
Staffing and referrals
Twenty-four-hour staffing and support are available at our CRS homes according to each resident’s assessed needs and service plan. This does not mean an RN is physically present in every home around the clock.
Referrals may come from case managers, care coordinators, counties, or other authorized sources. Final admission depends on eligibility, authorization, assessment, compatibility, and availability.
